When calling to order lamps, please specify your:
  • Model number, name and manufacturer of bed
  • Original lamp found on a bed exposure label
  • How many lamps and the wattage
  • Type of lamp end (rdc or bi pin)
  • How many facial lamps (High Pressure or Spaghetti) and the wattage

IMPORTANT INFORMATIONABOUT THIS SHIPMENT AND FUTURE SHIPMENTS.
Knowing this information may save you money!

UPS Shipments:

  • NEVER refuse a package even if there is obvious damage.
  • You must inform the driver of any damage and it must be noted either verbally or electronically.
  • Call your sales rep IMMEDIATELY to inform them of the damage or loss.
  • Your rep will ask for the condition of the package (crushed, smashed, punctured, leakage, etc). Keep the container and packing material for up to 10 business days. The shipping company may schedule an inspection.

If you have any questions do not hesitate to contact your sales representative.

Trucking:

  • NEVER refuse a package even if there is obvious damage.
  • COUNT the number of boxes and compare to the delivery receipt.
  • EXAMINE each carton on all sides. If the carton is damaged you have the right to inspect the contents of the carton.
  • Inform the driver of ANY damage. The damage MUST BE NOTED on the DELIVERY RECEIPT. (If not noted, it will be your responsibility to file a freight damage claim).
  • Call your sales rep immediately to inform them of the damage or loss.
  • Your rep will ask for the condition of the package (crushed, smashed, punctured, leakage, etc).
  • Keep the container and packing material for up to 10 business days.

The delivery drivers may tell you or your staff they cannot wait while you inspect your shipment for damage/s. If this is the case, note on the delivery receipt "POSSIBLE DAMAGE - DRIVER WOULD NOT WAIT FOR INSPECTION" and get the drivers full name.

If shipments are signed "clear" without damages being noted, the trucking company will assume the shipment was free of damage and will not accept any liability for damages. It is very important that you and your staff are aware of the proper procedure or it will cost you to have the damaged items replaced.
